Early warning systems cut European heat deaths but gaps remain
A study of 14 countries shows heat‑prevention plans slash mortality by a quarter, yet uneven implementation leaves vulnerable groups exposed.
Heat‑related mortality in Europe has risen sharply, with more than 200,000 deaths recorded over the past four years and an additional 35,000 fatalities linked to this summer's heatwaves. A new analysis covering 14 nations and almost three decades of data finds that coordinated prevention measures can reduce those deaths by roughly 25 percent.
What the evidence says
Researchers from the London School of Hygiene and Tropical Medicine and the Euro‑Mediterranean Center on Climate Change pooled mortality records, weather data and policy timelines to assess the impact of heat‑prevention programmes. The lead author, epidemiologist Antonio Gasparrini, described the result as "a very strong signal of effects of such interventions". The study shows that when early‑warning alerts, cooling shelters and systematic check‑ins are activated, the excess mortality that typically follows a heatwave falls noticeably.
Despite the clear overall benefit, the analysis could not pinpoint a single "best" intervention. "There wasn't strong evidence of difference in interventions," Gasparrini explained, noting that national plans differ widely in scope, timing and the mix of measures they employ. This diversity makes direct comparison difficult, but the aggregate picture is encouraging: coordinated action saves lives.
National approaches in practice
France remains a benchmark after the 2003 heatwave that claimed nearly 15,000 lives. The country introduced a tiered alert system that triggers national crisis centres, obliges regional authorities to open cooling sites, distribute water and conduct welfare checks on at‑risk residents. Spain followed a similar path, creating a Daily Mortality Monitoring system and establishing a network of free, air‑conditioned public buildings. In Barcelona, more than 500 cooling shelters are positioned so that 99 % of the population can reach one within ten minutes on foot.
These initiatives demonstrate political will, yet their real‑world uptake is still under scrutiny. Environmental physiologist Andreas Flouris of the University of Thessaly warned that "we don't know if they're being used". Without reliable data on shelter occupancy or the frequency of welfare checks, the health impact of such infrastructure remains partly speculative.
Cost‑effectiveness and the missing metric
Co‑author Shouro Dasgupta, a researcher at the Euro‑Mediterranean Center on Climate Change, argues that early warning systems deliver the greatest return on investment. When a temperature threshold is breached, automatic triggers launch cooling centres, issue health guidance and mobilise volunteers, all with minimal administrative lag.
Europe, however, has yet to settle on a common definition of that threshold. While most programmes default to a simple temperature reading, Flouris points out that the wet‑bulb globe temperature (WBGT), which incorporates humidity, solar radiation and wind, offers a far more accurate picture of how heat feels on the body. The EU‑funded HEAT‑SHIELD project highlighted the metric's relevance for occupational settings, where indoor humidity can make a modest temperature feel dangerously oppressive.
Targeting the right groups also matters. An evaluation of the Netherlands' national heatwave plan by the Dutch Institute for Public Health and the Environment found the strongest risk reductions among older adults, women and low‑income households. These findings echo a broader concern that many heat‑health action plans aim for universal coverage rather than focusing resources on those most vulnerable.
Vulnerable groups left behind
"Most heat health action plans are designed to reach as many people as possible, not the people most at risk," Dasgupta said. The widening gap between climate‑induced hazards and social inequality means that the elderly, people with chronic illnesses and households that cannot afford air‑conditioning remain exposed.
Paris has responded by offering an opt‑in registry for seniors and other vulnerable residents, while Denmark relies on neighbourhood volunteers to check on neighbours during extreme heat. Yet these schemes are patchwork solutions that depend on personal initiative rather than systematic protection.
Workers, especially those outdoors or in poorly ventilated factories, face a different set of challenges. General advice to "stay indoors, drink water and slow down" does not translate to a construction site or a warehouse. Flouris' research indicates a productivity loss of about 2.6 % for each degree Celsius above a WBGT of 24 °C, a 15 % drop at 30 °C WBGT. He recommends three low‑cost measures for employers: provide ample drinking water, schedule regular rest breaks and ensure shaded rest areas. Such steps could safeguard health while preserving output.
Beyond reactive measures
Professor Ronita Bardhan of the University of Cambridge argues that the current focus on reactive alerts overlooks a more fundamental solution: redesigning the built environment. "People experience heatwaves through their homes, through their workplaces," she notes, urging retrofits that improve insulation, ventilation and shading. Rather than asking citizens to change behaviour only during a crisis, structural interventions could lower indoor temperatures year‑round, reducing reliance on emergency shelters.
Bardhan's call for "environmental interventions that produce an environment that reduces exposure to the heat" aligns with a growing European agenda on climate‑resilient housing. The European Commission's recent "Fit‑for‑55" package includes provisions for energy‑efficient retrofits, but funding mechanisms and implementation timelines remain uneven across member states.
Looking ahead
The study's authors stress that while early warning systems and cooling shelters are proven tools, Europe must harmonise thresholds, improve data collection on usage, and tailor interventions to the most exposed groups. Without such refinements, the mortality gap is likely to widen as heatwaves become more frequent and intense.
For trade unions and workers' organisations, the message is clear: heat is not only a public‑health issue but also an industrial one. Protecting labourers from heat stress can preserve productivity and prevent a hidden cost that often falls on workers' health benefits. Collective bargaining agreements that embed heat‑risk assessments, mandatory rest periods and provision of cooling equipment could become a new frontier in workplace safety.
Policy makers, city planners and employers now face a choice. They can continue to rely on short‑term, reactive measures that save lives in the moment, or they can invest in longer‑term, structural changes that level the playing field for the most vulnerable. The evidence suggests that a blend of both, robust early alerts paired with retrofitted homes and workplaces, will be essential to curb the rising death toll from Europe's scorching summers.
